마연주 한지혜 2022-08 32068 https://aurora.ajou.ac.kr/handle/2018.oak/20910 학위논문(석사)--아주대학교 임상치의학대학원 :임상치의학과,2022. 8 서 론 : 이 연구는 Cone-Beam Computed Tomography(CBCT)와 파노라마방사선 영상을 이용하여 상악 매복 견치의 견인 치료기간에 영향을 미치는 요인을 알아보고자 하였다. 대상 및 방법 : 2012년 1월부터 2020년 12월까지 아주대학교 치과병원에 내원한 환자들 중 상악 견치의 매복으로 진단받아 외과적 노출 및 교정적 견인치료를 시행 받고 상악 매복 견치의 맹출까지 치료 받은 만 8세이상 만 18세이하 환자 73명(93개의 상악 매복 견치)의 의무기록지와 CBCT 영상 및 파노라마방사선 영상을 분석하였다. 결 과: 교합평면에서 상악 매복 견치의 교두정까지의 거리와 환자의 나이는 견인 치료기간과 양의 상관관계를 보였다. 반면 교합평면과 매복 견치의 치축이 이루는 각도와 편측성/양측성 매복 양상은 견인기간과 음의 상관관계를 보였고, 이전 연구와 달리 이번 연구에서는 상악 견치의 양측성 매복을 보일 때 견인 치료기간이 더 짧게 나타났다. 환자의 성별, 상악 매복 견치의 근-원심 위치, 협-구개 위치, 상악 견치의 치근 성숙도와 만곡의 유무는 견인기간과 상관관계를 보이지 않았다. 결 론: 진단 시 촬영하는 CBCT 영상과 파노라마방사선 영상을 이용한 상악 매복 견치의 매복 양상에 대한 분석을 통하여 치료시작 시 환자의 어린 나이, 낮은 매복 깊이, 매복 견치의 양호한 치축, 구개 확장 장치의 사용 등이 상악 매복 견치의 견인기간을 짧게 예측하는데 도움을 줄 수 있을 것이다. Materials and Methods: This study was performed by reviewing medical records and radiographs of 73 patients (93 impacted maxillary canines) with the age of 8 to 18 years, who were treated with surgical exposure and forced eruption of impacted maxillary canines. Results: The results showed positive correlation of the treatment duration with the distance between the canine cusp tip and the occlusal plane and also with patient’s age. There was negative correlation of the treatment duration with the angulation between the axis of canine and the occlusal plane and unilateral or bilateral impaction group; patients with unilateral impaction required longer treatment duration than that of patients with bilateral impaction. No significant correlation was found with other variables, including gender, mesio-distal location, bucco-palatal location, development of root and root dilaceration. Conclusions: Patient’s age, distance from the canine cusp tip to the occlusal plane, angulation between the axis of canine and the occlusal plane, and type of impaction allow us to predict treatment duration of forced eruption in impacted maxillary canines and to provide information about planned treatment to patients.
