이은소 김진철 2020-08 30327 https://aurora.ajou.ac.kr/handle/2018.oak/19778 학위논문(석사)--아주대학교 일반대학원 :의학과,2020. 8 연구배경: 베체트병 (Behçet’s disease)은 재발성 점막피부 병변 및 안구 증상을 비롯한 다른 전신 장기를 침범을 특징으로 하는 만성 염증성 질환으로, 동부 및 중앙 아시아 (Eastern and Central Asian) 및 동무 지중해 국가 (Eastern Mediterranean countries)에서 가장 유병률이 높은 것으로 알려져 있다. 베체트병의 발생 기전에 대해서 활발히 논의되고 있으나, 명확한 기전은 밝혀지지 않았다. 최근 연구 결과에 따르면, 베체트병을 비롯한 여러 결합조직질환 (connective tissue disease)에서 정상 대조군과 비교하여 마이크로바이옴 (microbiome) 구성 (composition)의 변화가 관찰되었으며, 이는 면역 기능 장애 (immune dysfunction)와 연관하여 질병 발생에 영향을 줄 수 있다고 언급하고 있다. 하지만, 한국인 베체트병 환자에서 마이크로바이옴의 역할에 대한 연구는 없었다. 연구목적: 따라서, 본 연구에서는 베체트병 환자, 재발성 아프타성 궤양 (recurrent aphthous ulcer) 환자, 정상 대조군에서의 대변 및 타액 마이크로바이옴 분석을 통해 어떠한 차이가 있는지 비교하였으며, 질병 활성도에 따라서 어떠한 변화가 있는지를 확인하고자 하였다. 연구방법: 베체트병 환자 (N=7), 재발성 아프타성 궤양 환자 (N=7), 각각의 환자와 적어도 하루 한끼 이상의 식사를 같이 하는 정상 대조군의 대변 및 타액에서 DNA 추출, PCR 증폭 및 염기서열분석을 과정을 통해 얻은 조작분류단위 (operational taxonomic units)를 이용하여 알파 다양성 (alpha diversity), 베타 다양성 (beta diversity), 그리고 상대존재비 (relative abundance) 등에 대한 마이크로바이옴 분석을 시행하였다. 연구결과: 베체트병 환자에서 대조군에 비해 높은 마이크로바이옴의 풍부성 (richness) 및 균등성 (evenness)을 보였으나, 환자군 및 대조군 사이의 알파 다양성의 유의한 차이는 없었다. 베타 다양성은 오직 베체트병 환자와 대조군 사이에서만 유의한 차이를 보였다. 상대존재비 분석에서는 정상 대조군에 비해 베체트병 환자의 대변에서 Bacteroides uniformis가 증가해 있었고, 타액에서 Rhothia mucilaginosa가 감소해 있었다. 그리고 재발성 아프타성 궤양 환자의 대변에서 Lachnospiraceae 과 (family)에 속하는 종 (species) 및 속 (genera)들이 정상 대조군에 비해 감소해 있었다. 질병 대조군인 재발성 아프타성 궤양 환자의 대변에서 Bacteroides dorei가 증가해 있었다. 질병 활성기에 따른 분석에서 비활성기 베체트병 환자에 비해 활성기 베체트병 환자의 대변에서 Firmicutes 속이 증가해 있었고, 타액에서 Actinobacteria 속이 증가해 있었다. 포도막염 (uveitis)의 유무에 따른 분석에서 포도막염이 없는 베체트병 환자에 비해 포도막염이 있는 베체트병 환자 대변에서 Bifidobacterium adolescentis가 증가해 있었다. 연구결론: 본 연구는 마이크로바이옴의 한국 베체트병 환자에서의 역할에 대해 분석한 최초의 연구이다. 본 연구를 통해 베체트병 환자의 대변 및 타액의 마이크로바이옴 불균형 (dysbiosis)이 관찰되지는 않았으나, 정상 대조군이나 재발성 아프타성 궤양 환자와 비교했을 때 마이크로바이옴 구성의 변화 (alteration)가 있음을 확인하였고, 이러한 마이크로바이옴의 변화가 베체트병의 발생 혹은 질병 활성에 영향을 줄 수 있을 것이며 이에 대한 조절을 통해 치료적 효과를 기대 해 볼 수 있을 것으로 생각된다. 하지만 더 의미 있는 결과를 얻기 위해 더 큰 규모의 전향적 연구를 통한 검증이 필요하다. Although the pathogenesis of BD has been actively discussed now, it is largely unknown. Recent studies have revealed that there are compositional changes in fecal or salivary microbial communities in patients with connective tissue disease, including BD, compared with healthy controls, and those are thought to be associated with immune dysfunction. However, studies on whether the microbiome plays an important role in Korean patients with BD have not been carried out. Therefore, in this study, we investigated the fecal and salivary microbiome in patients with BD, patients with recurrent aphthous ulcer (RAU), as disease controls, and healthy controls (HCs), and compared how different the microbiota taxonomic compositions of those groups and analyzed whether there was a changes in the microbiome composition according to disease activity. Material and methods: Fecal and salivary samples were collected from patients with BD (N=7) and RAU (N=7) and BD-matched HCs and RAU-matched HCs. HCs were selected from among healthy family members of each patient who had been eating one or more meals per day with the patient. Full genetic analyses of microbiomes in all the samples were performed through DNA extraction, followed by polymerase chain reaction amplification and sequencing. Finally, operational taxonomic units (OTUs) with single reads were used for a secondary analysis, which included alpha diversity (number of OTUs, Chao1 and Shannon diversity indices), beta diversity (Bray-Curtis principal coordinate analysis plot), and taxonomic biomarkers (linear discriminant analysis effect size algorithm). Results: There were no significant differences in alpha diversity between the disease groups and disease-matched HC group, although the active BD group showed a higher richness and evenness of microbiome taxonomic composition than the BD-matched HC group. However, beta diversity was significantly different between only the BD and BD-matched HC groups. In relative abundance analyses, the active BD group had a significantly increased abundance of Bacteroides uniformis in stool and decreased abundance of Rothia mucilaginosa group in saliva compared with the BD-matched HC group. Additionally, most of the abundant taxa in the stool of active RAU patients were species and genera belonging to the family Lachnospiraceae. We also showed differences in the abundance of Bacteroides dorei between the active BD and active RAU groups. In the subgroup analysis according to disease activity and underlying uveitis, the active BD group displayed significantly higher abundances of the phyla Firmicutes and Actinobacteria in stool and saliva, respectively, than the inactive BD group, and the active BD group with uveitis showed a significantly higher abundance of fecal Bifidobacterium adolescentis than the active BD group without uveitis. Conclusion: This is the first Korean study to investigate whether microbiome plays important roles in patients with BD. In this study, although significant dysbiosis of fecal and salivary microbiome in BD patients compared with those of normal subjects was not noted, several compositional changes were found, and these microbiomes could affect the BD occurrence or pathogenesis and the systemic progression of the BD. Although alteration of oral and gut microbiome composition in patients with BD is thought to be related to the pathophysiology of BD, and the treatment effect can be expected by controlling these alterations in bacterial microbiome, further prospective studies with a large sample size are needed to obtain more significant results.
